AWARENESS AND ASSUMPTION OF RISK
 I am aware that this event involves risks of personal injury, death, damage to property, expense and related loss including loss of income. Included in these risks are negligence on the part of the organizer of Sultan Ahmad Shah (SAS) International Eco-Challenge, its Directors, Officials and Volunteers, other participants, owners of the facilities where the events occurs, sponsors and others involved in this event. I freely accept and fully assume all such risks and the possibility of personal injury, death, damage or loss to my personal property, expense and related loss, including loss of income.

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY
 In consideration of Tentu Intan Sdn Bhd, the organizer of this event, accepting my application to participate in this activity, I agree:
1. To waive any and all claims I may have in future against Tentu Intan Sdn Bhd and others involved in this event.
2. To release Tentu Intan Sdn Bhd and others involved in this event from any and all liability for any personal injury, death, damage to my personal property, expense and related loss, including loss of income that I or my next of kin may suffer as a result of participation in this event due to cause whatsoever, including negligence, breach of contract or breach of any statutory duty of care.
3. To hold harmless and indemnify Tentu Intan Sdn Bhd and others involved in this event from any and all liability of any damage to property of, or personal injury to, any third party, resulting from my participation in this activity.
4. That this agreement is binding on not only myself but my next of kin, heirs, executors, administrators and assigns.
I HAVE READ THIS AGREEMENT AND UNDERSTOOD IT. I AM AWARE THAT BY SIGNING THIS DOCUMENT I AM WAIVING CERTAIN RIGHTS WHICH I OR MY NEXT OF KIN, HEIRS, EXECUTORS, ADMINISTRATORS AND ASSIGNS MAY HAVE AGAINST TENTU INTAN SDN BHD AND OTHERS INVOLVED IN THIS EVENT. I ALSO HEREBY DECLARE THAT I AM MEDICALLY FIT.

I hereby understood, agree and give my consent to the above Awareness & Assumption of Risk / Release of Liability, Waiver of Claims and Indemnity.
